Grade 1 students please turn in the copy of "VACCINATION RECORD" by Sept 13.

Dear Grade 1 students and parents, 

          The health center needs to check Grade 1 students' vaccination record.  Please hand in your child's vaccination record to the homeroom teacher.  Please turn in the COPY only.   The sanitation section will collect all the copies of vaccination for 1A and 1B homeroom teachers by this coming Friday. (Sept. 13)

          Please ignore this message if you already gave it to the homeroom teachers or sanitation section.
